If every lead is worth the same to Google Ads, the algorithm will find the cheapest ones. For a hospital that sells both a general orthopaedic consultation and a knee replacement, that is the wrong instruction. The consultation is common, cheap and often resolves without surgery. The replacement is rare, high-value and takes weeks to close. A bidding system told that both are "one lead" will chase whichever is easiest to generate.
This article is for surgeons, hospital marketing leads and the agencies who run their accounts. It explains how to assign conversion values from procedure margin and close rate, how to import those values into Google Ads, and how to do so without sending patient health data to an advertising platform. It assumes you already track form fills and calls. For the broader context on ortho marketing, see our guide to attracting joint replacement patients.
The numbers in this post are hypothetical and exist only to show the arithmetic. Your finance team must supply the real margins.
What value-based bidding needs
Google's Target ROAS documentation states the requirements. You must set values for the conversions you track. For Search campaigns, it asks for at least 15 conversions in the past 30 days at the conversion-tracking level. Before setting a target, it advises reporting values for four weeks or one to two conversion cycles, whichever is longer, and notes that you can start by bidding to your goal with Target CPA.
Two consequences follow. A hospital with long decision cycles may need to wait several weeks before the value data is meaningful. And a hospital with only a handful of surgical conversions per month may not meet the volume threshold on the surgical action alone, which is why the design below uses staged values.
Step 1: Put a rupee value on each stage
A knee replacement is not a single event. It is a sequence: enquiry, qualified lead, consultation booked, consultation attended, surgery advised, surgery done. The right value to give each stage is the expected margin that stage will eventually produce.
The formula for a stage is:
Stage value = probability of reaching surgery from this stage x contribution margin per surgery
Contribution margin should be what the hospital keeps after the direct cost of the procedure, not the bill value. Finance owns that figure. Use a rounded, conservative number and update it quarterly.
A hypothetical worked example
Assume the hospital's finance team gives a contribution margin of Rs 60,000 per primary knee replacement. Assume the following conversion rates, observed over a year of CRM data. These figures are invented.
| Stage | Chance of reaching surgery from here | Stage value |
|---|---|---|
| Raw enquiry (form or call) | 1 in 50 | Rs 1,200 |
| Qualified enquiry (right condition, right city) | 1 in 15 | Rs 4,000 |
| Consultation attended | 3 in 10 | Rs 18,000 |
| Surgery advised and accepted | 8 in 10 | Rs 48,000 |
| Surgery completed | Certain | Rs 60,000 |
A general orthopaedic enquiry about an unrelated issue would sit lower. A consultation for a condition that rarely leads to surgery might be worth Rs 2,000 rather than Rs 18,000. The point is that values differ by procedure, not that these particular numbers are right.
Do not add the stages together. If a patient passes through all five stages, you report each stage as a separate conversion, and the total credited would exceed the margin. Either upload only the most advanced stage as the primary conversion, or mark earlier stages as secondary so they do not count toward bidding. Google Ads allows conversion actions to be set as primary or secondary for this purpose.
Step 2: Choose which stages Google can actually see
Online conversions (a form fill, a click-to-call) are available immediately. Later stages happen in your CRM. You have to bring them back. Google documents the mechanism as offline conversion import and, for lead data, enhanced conversions for leads. Both match a later outcome to the original ad click. Google's help page also says that offline imports and enhanced conversions for leads uploads are moving to its Data Manager from June 2026, so check the current setup path before building.
Our post on offline conversion tracking for clinics covers the plumbing, and the Pioneer CRM demo shows one way to hold the stage data. The decision here is which stages to send.
A workable pattern for most orthopaedic accounts:
- Primary action, volume driver: qualified enquiry. It occurs often enough to meet the 15-conversion guideline and the value is meaningful.
- Primary action, quality signal: consultation attended, imported from the CRM within days.
- Secondary or reporting only: surgery advised and surgery completed. They happen too rarely to steer a daily bidding model, but they matter for checking whether your values are right.
Step 3: Keep patient data out of the signal
The value signal does not need health information. Design the import so that it carries only what Google needs to match the click and apply a number.
- Send: click ID or hashed contact identifiers as the documented method requires, the conversion time, the conversion action name and a value.
- Do not send: diagnosis, procedure name in free text, doctor name, clinical notes, imaging findings, or any custom field containing health detail.
- Name conversion actions neutrally, for example "Qualified enquiry - tier 2". Keep the mapping from tier to procedure inside your own systems.
- Bucket values. Round to a small set of tiers so a value cannot be read as an individual's bill.
- Do not put health data in URLs or form fields passed to ad tags.
Google's personalised advertising policy treats health as a sensitive category with targeting restrictions and prohibits ads that collect personally identifiable information. Note that Google does not sign business associate agreements for HIPAA, so the safe design is to avoid sending protected information at all. In India, consider your obligations under the Digital Personal Data Protection Act, and have counsel review consent wording on the form. Our compliance overview explains the boundaries.
