Instagram shipped two changes in late 2025 and early 2026 that fundamentally alter how doctor accounts grow. If you're still optimizing for likes and follower count, you're playing last year's game.

Change 1: "Your Algorithm" Gives Users Direct Control

Launched December 2025, "Your Algorithm" lets users see and edit the topics Instagram thinks they care about. Users can add topics ("dermatology," "mental health," "orthopedics") or suppress topics they don't want.

What this means for doctors: Your content now needs to align with clearly defined medical topic categories, not just engagement metrics. If a user has "cardiology" as an interest, your cardiology content will reach them even with low initial engagement — IF Instagram correctly categorizes your content.

Change 2: Saves and Rewatches Now Outrank Likes

In 2026, the ranking signal hierarchy is: Saves > Shares > Rewatches > Comments > Likes. Instagram isn't rewarding popularity anymore — it's rewarding utility. Content that people save for later reference performs 3-5x better than content that just gets double-tapped.

The New Doctor Content Playbook

Format 1: Saveable Carousel Infographics
Create 5-7 slide carousels explaining conditions, treatment options, or recovery timelines. These get saved at 4x the rate of video content because patients want to reference them later. Include your clinic name on every slide.

Format 2: 60-90 Second Educational Reels
Short Reels (under 30 seconds) are losing ground. Instagram now favors 60-90 second Reels that maintain high completion rates. For doctors, this is perfect: explain one condition, one myth, or one procedure in 60-90 seconds. Reels get 2.35x more reach than static posts and are shown to non-followers.

Format 3: Raw, Unpolished "Day in the Life" Stories
As AI-generated content floods Instagram, authentic human content stands out more than ever. Patients want to see the real person behind the white coat. A 15-second Story of you walking into the hospital with a coffee is more relatable than a polished branded video.

Format 4: Trending Health Topic Reactions
When a health topic trends — a celebrity diagnosis, a new study, a viral health myth — doctors who react within 24 hours get massive algorithmic boost. Set up Google Alerts for keywords in your specialty.

What to Stop Doing

Stop using 30 hashtags. Hashtags are now just context indicators, not discovery tools. Stop buying followers. Instagram's new algorithm aggressively down-ranks accounts with suspicious engagement patterns. Stop posting only promotional content. The 80/20 rule (80% educational, 20% promotional) is now more like 90/10.
